PeriANAL lac or PeriNEAL lac? If it's the former, I think the blood is explained. The anus is 4 cm long, do you have an anal canal injury? Sounds like an aweful lot of blood to becoming from the examining finger of the ED physician. How high up is the blood from the anal verge? If you did a wash out and there was still blood, you either have a distal rectal or, anal injury. Was there an associated pubic ramus fx? What did the vaginal exam reveal?
