I really cannot see how politics/culture/customs has anything to do with this issue--either there is a valid benefit to it or not--this is a typical dodge of an argument from people who just want to use a toy but cannot justify it. The basic tenets of science do not change according to the country or trauma system you are in--if you see a benefit, please just demonstrate it or cite it to us, rather than continuing with these evasive and irrelevant ancdotes and testimonials ERF -----Original Message----- From: Coats Tim - Professor of Emergency Medicine <Tim.Coats at uhl-tr.nhs.uk> To: Trauma & Critical Care mailing list <trauma-list at trauma.org> Sent: Wed, 18 May 2005 12:53:30 +0100 Subject: RE: prehospital FAST There seems to be a Europe / USA split on the pre-hospital FAST. (Not used in the UK). Is there something about trauma system organisation in Scandanavia and Germany that makes early identification of intra-abdominal bleeding more useful? In the UK surgeons (sweeping generalisation) are becoming less interested in trauma management and less active in the early phases of trauma care (Reasons are increased surgical specialisation and shortened training giving narrower experience). Subtle changes such as these might change the value of a pre-hospital FAST - for example in a sytem without designated trauma centres it might allow an appropriate choice of hospital destination. I'm not saying that this is true - but to say that the patient is going to be taken to a trauma centre anyway does not take account of the differing systems of trauma care around the world.
