I have heard of patients who have died because of lack of continuity of care, as a matter of fact Libby Zion was such a case. The medical resident that did not Ms Zion, and gave a verbal order used fatigue as her defense. WRONG. I see problems every month in Sentinel Event Committee reviews caused because of multiple handoffs of the patient. Continuity of care. I have not seen a single case regarding fatigue in the sentinel event process. I keep hearing about doctors falling asleep and hurting themselves at the wheel of a car. I am aware of such cases after drinking at a social event, but I am really not aware of such a case following a physicians call. I am not aware of a surgeon falling asleep at the operating table. Please we need a little EBM here. Provide the data for the claim made above. k
