Jeff,

There's actually quite a bit of info on this subject available directly from the internet.  Just do a GOOGLE search on "high altitude sickness."  Much of the original research on high altitude sickness has been done by military physicians for obvious reasons: it can affect military personnel fighting or working at high altitudes and of course pilots are sometimes exposed to these conditions in unpressurized airplanes.  High-flying unpressurized planes are used more often in the military than most people realize...such as in making supply drops out the back of C-130's, etc.

I happen to be a skydiver and have read extensively on the subject of the effects of high altitude on people but have not done any original research in the field.  I have also done several high altitude skydives, including one from 31,000 feet altitude, which obviously required oxygen on the ride to altitude and oxygen bail-out bottles.  Since we need to open the door to make the skydive, these planes cannot be pressurized.
