Prehospital CPR for trauma more than 4 minutes = no NOTHING upon arrival at EC. Blunt trauma and cardiac cessation at time of arrival in ED. EC thoracotomy on PROTOCOL and by surgeon available who know what to expect, what to do, and which incision to attempt will have an occassional save. Less than 3%, probably less than 1% survival. However, a blunt rupture of atrium, cardiac herniation, and a rare other injury might be repaired via an emergency center thoracotomy. Remember Princess Di, had at least 2 pre hospital cardiac cessation episodes in the 140 minutes between the time of her accident and her arrival at the hospital. She underwent emergency center thoracotomy. k
