Actually, I just saw a copy on the shelf at U of Washington bookstore (Seattle).

North American release yet. Can someone recommend a reputable online
source?

I second this. I am an emergency physician and in lieu of trauma surgeons
existing in the UK I am often the person coordinating the initial
(non-operative) management of these patients and theatre triage in
conjuction with my surgical colleague.

I have a very large collection of trauma and emergency medicine books and
this is by far and away the best written and practical of all the trauma
surgery books currently available. It is obvious that it is written by
people who have "been there, done that" to help those who have less
experience. Unusually it manages to do this while being non-patronising and
genuinely funny too.

Very well done Ken and Ascher.

I have recommended this to every surgeon who ventures into our ED.
