Near Drowning

Hi Marc,

This is our local policy. Every patient rescued for near-drowning,
regardless of initial status is brought to our emergency room.Patients with
respiratory symptoms are admitted in a pediatric, medical, or critical car
ward according to data gathered from physical exam, chest X-ray, blood gases
etc. Patients with no symptoms are admitted in a short duration unit and
monitored overnight. 
We do not have a strong triage rule, and I believe such a situation to be
difficult to anticipate. I found a paper [Causey AL, Tilelli JA, Swanson ME
Predicting discharge in uncomplicated near-drowning Am J Emerg Med
2000;18:9-11]on that topic that could be helpful.

Bierens, a prof. Emergency Medicine who wrote his thesis on (near) drowning,
suggested that these near drowning victums, who appear ok, should be taken
to a hospital for a phisical, X-chest, and ABG without extra oxygen.  If
this is normal, a 6 hour observation on a ward and repeat tests. Still
normal sent home.
