Actually, the pleura is complex.   I can tell you from bitter 
experience 
that opening the diaphragm for a washout, simply does not  work.   The
empyema 
rate is much higher compared to a formal  posteriolateraly thorcotomy
which 
allows for removal of all the foreign bodies,  food, etc.
