A good topic--actually, all accredited hospitals in the states are required to eliminate all but the most common and accepted acronyms/abbreviations from the medical record by JCAHO, but it continues to amaze me how they just continue to seep in.  And it cannot be explained by anything but galling arrogance that someone can write such abbreviations in the chart that nobody but them have any idea as to its meaning, obviously without the slightest inkling that others read these records other than they--to give an expample--a resident wrote the following abbreviation in the H&P of a trauma patient just the other night, and was aghast that nobody besides her alone--her attending with 25 years of experience, any of the experienced trauma nurses, any other resident, the janitor, etc--had the slightest idea what it meant--"PHBC"
Anyone hazard a guess as to what she meant by this, to show me what a klutz I must be not to have seen it right off?
