THe liver is injured with tube thoracostomy much more often than all of us  
on this list server would like to admit.   It is more common  with:
 
1.    Tubes placed too low
2.    By persons who dont really know or understand  anatomy
3.    When any kiind of TROCAR is used.  
 
TROCARS as part of chest tubes should be removed from all products used in  
people, as they have repeatedly been demonstrated to be a health hazard.   The 
members of this list server have documented and supported the non-use of  
trocar chest tubes more times than I care to recount
