I don't see a lot of nausea but I feel damn sorry for those who do turn green on me, and would like to do my best to prevent it. What do you do with those who do develop nausea?

Since we very rarely see nausea with careful titrations of morphine or fentanyl (our analgesics of choice in conscious patients), there's no compelling indication for prophylactic antiemetics.
If for some reason I must give meperidine (some of our clinicians are--well, old), I'll ask to give a whiff of promethazine with it. But most of our patients do fine without unnecessarily complicating their pharmacology.
Is your experience otherwise?
