Hi 

I would insert RIGHT intercostal drain, maybe a LEFT as well.

Emergent laparotomy - control bleeding

Subxiphoid window and proceed if necessary

Depending on chest drain output - thoracotomy may be indicated

Complete surgery with on-table rigid or flexible scope to exclude 
thoracic oesophageal injury
