AVPU was part of the original ATLS course. I took my first ATLS provider
course in 1984 at the University of Nebraska. There was no manual, only a
bunch of loose leaf typed papers. AVPU was there.  GCS was part of the
secondary survey. 

As I wrote earlier, AVPU was never intended to replace the GCS.
