Like so many in the UK, as a result of the government dictat, you ( and I,
no criticism intended of you ) are re-inventing the wheel. We discussed this
recently, and I would be at a meeting about this on  Monday, but I decided
the charms of the south of France were better :-)) Currently, we use the
documentation produced by University College London, who do far and away the
most adult transfers in this neck of the woods. They were happy to send it
to me, but its on my work PC. I suggest you e-mail Debbie Cook (
debbiecook.ncls@virgin.net ) and ask her for a copy.

    I am currently looking into the development of a transfer sheet, to be
used in the transfer of patients from A&E to ITU etc which may be at another
hospital.  I have sent letters to 20 hospitals around the UK but had only 1
reply - please help if you can.
