penetrating definition draft 1

The only reason at all to fret over this is for the sake of standardization.
If we intend to compare populations, which is forever something we're wont
to do, then it only makes sense to specify definitions.

This is really a simple matter to automate:  If the final principle
diagnosis has an ICD code which specifies an "open" injury, then it's
penetrating trauma.  Objective, international, reliable.  Also happens to be
the way most mature registries classify these patients.  Why reinvent the
wheel?

Primary or major injury is the puncturing or piercing of the skin, cavity or
deeper organs by a missile or sharp foreign body
