Hi Eitan. Massive hemothorax , severe lung contusion, or pericardial tamponade are all possible etiologies for respiratory distress in a patient with GSW to the chest. I think that one of the biggest problem with needle application is the false sense of security. There is a misconception (which is supported by the ATLS), that needle appliocation solve the problem of TP, at least temporarily. Except for the fact that there is no evidence to support needle application, at least half of them are found to be outside the pleural cavity anyhow.
